About the Role
We are seeking a Clinical Social Worker to become a part of our team! In this role, you will:
Provide individuals, families, and groups with the support needed to cope with chronic, acute, or terminal illnesses.
Collaborate with medical professionals to evaluate patients' medical or physical conditions and to assess client needs.
Refer patients to community resources to assist in recovery.
Investigate child abuse cases and take authorized protective action when necessary.
Counsel patients to help them overcome dependencies, recover from illness, and adjust to life.
Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
Modify client treatment plans to comply with any changes in status.
Organize support groups or counsel family members to help them understand, deal with, and support the patient.
Attend and participate in staff development programs, department related conferences and administrative meetings.

Requirements
Master's degree
Appropriate certifications and/or licensures with at least 5 years of related experience
Excellent verbal and written communication
